29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: At the time of the serious injury on 8-9-2015, two employees were not utilizing the correct lockout procedure for the #18 Plating Crane. Two employees placed locks on a remote box near the area where the maintenance was to be conducted. Placing locks on the remote box had become allowed procedure. The employer had installed locking devices on the remote boxes.
