29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regularly, and maintained in a safe condition: At 2055 Progress Drive in Farmington, MO 63640 (Plating Area) - On or about 08/27/2019, employees were exposed to the hazard of tripping and falling to same level where the walking and working surface in the plating area was loose and had several holes in the fiberglass type flooring, and was not maintained in a safe condition.
