OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed-by hazards:     a) K-Plant, press area, 6th and Center Streets, Tarentum, Pennsylvania 15084:  Observed on or about July 31, 2015, the emergency stop on a Laeis Bucher HPF 1400 hydraulic press (# 2 press) was not functional.  The emergency stop button was broken off of the panel and taped over.  Press operators and others working in the area were exposed to crushed-by hazards due to possible machine process malfunction.      b) K-Plant, press area, 6th and Center Streets, Tarentum, Pennsylvania 15084:  Observed on or about July 31, 2015, the emergency stop on a Laeis Bucher HVAKP 2000 hydraulic press (# 3 press) was not functional. The #2 press motor control cabinet was labeled as a HRC Level 4 arc flash and shock hazard.  Press operators, electricians, forklift operators, and laborers working in the area of the #2 press PLC cabinet were exposed to the hazards of arc flash, electric shock and electrocution.      b) K-Plant, electrical panels behind the hydraulic presses, 6th and Center Streets, Tarentum, Pennsylvania 15084:  On or about July 28, 2015, a significant amount of graphite dust accumulated inside the '2C' panel on the #1 Press motor control cabinet.  This panel was left open due to a broken clip.  The #1 Press motor control center houses 480 volt, 3 phase conductors and was labeled as a HRC Level 4 arc flash and shock hazard.  Press operators, electricians, forklift operators, and laborers working in the area of the #1 press motor control cabinet were exposed to the hazards of arc flash, electric shock and electrocution.
