OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  (a)  In the warehouse east wall:  On August 13, 2015 and at times prior to; the employer exposed his employees to struck-by hazards from stored inventory and or steel rack sections, in that employees were allowed to walk and work around steel racks columns (7 of 10) that were damaged at the base.  (b)  In the warehouse west wall:  On August 13, 2015 and at times prior to; the employer exposed his employees to struck-by hazards from stored inventory and or steel rack sections, in that employees were allowed to walk and work around a steel rack column that was damaged at the base.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  (a)  On the production floor:  On August 13, 2015 and at times prior to; the employer exposed maintenance personnel to pneumatic pressures of 90 psi and hydraulic pressures in that the LOTO procedures only identified the electrical power source of 230V for all 20 of the injection molding machines 1-10.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  (a)  In the control loading area: On August 13, 2015 and at times prior to; the employer exposed employee to electrical hazards in that employees were allowed to work and walk through areas where extension cords were used as permanent wiring to operate the two gravimetric blenders.  (b)  On the production floor: On August 13, 2015 and at times prior to; the employer exposed employee to electrical hazards in that employees were allowed to work and walk through areas where extension cords were used as permanent wiring to operate the conveyer on injection molding machine #10.
