29 CFR 1910.178(l)(3)(ii)(A): Powered industrial truck operators did not receive initial training in surface conditions where the vehicle would be operated that could affect safe operation: At Cumberland Plastic Systems: On or about June 12, 2018 and at times prior; the employer exposed its employees to crush and struck-by hazards in that employees were not given additional training on operating Powered industrial truck inside trailer to include but not limited to locking trailers to the docks and installing wheel chocks on trailer to prevent movement. On May 30, 2018 an employee was loading a trailer with a forklift and while in the trailer the truck operator pulled off with the trailer causing the powered industrial truck to fall out of the trailer with the employee in it. The trailer was locked into the loading dock and wheels on the trailer were not chocked.
