29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about August 11, 2015, lock out procedures were not developed or used for the control of hazardous energy on the Farrel Head Stretcher while employees performed maintenance operations, exposing employees to hazards such as but not limited to, being caught in or struck by moving parts. The Ply Gem Windows DBA New Alenco Extrusion, Ltd., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(I), which was contained in OSHA inspection number 979875, citation number 1, item number 2a and was affirmed as a final order on 08/18/2014 with a final abatement date of 08/12/2014 with respect to a workplace located at 615 W. Carson St, Bryan, TX 77801.
