29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    a) 300 Charles St., Lindenhurst, NY - Lindenhurst High School: Employees were working on a scaffold, four sections high and set atop a building. Employees were working on platforms having areas, ranging from ten inches to two feet, without planks; on, or about, 08/17/2015.         Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) 300 Charles St., Lindenhurst, NY - Lindenhurst High School: Employees on a roof, approximately 18 feet in height, were engaged in work activity without protection from falling; on, or about, 08/17/2015.       REPEAT VIOLATION: Marfi Contracting Corp. was previously cited for a violation of this Occupational Safety and Health Standard, its equivalent, or essentially similar standard 29 CFR 1926.501(b)(10), which was contained in OSHA Inspection Number 984800, Citation #1, Item #1, issued on 09/26/2014, and became Final Order on 10/24/2014.      Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
