Location: a) 2 story building; adjacent to building labeled R109.   a) On or about 09/03/2020, an employee walked on a roof to perform patching of holes  and the roof was not inspected by a competent person.  An employee fell approximately 25 ft. through the roof to the interior of the building.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  Location: a) 2 story building; adjacent to building labeled R109.   a) On or about 09/03/2020, an employee walked near the edge of the roof without the use of fall protection. Employee was exposed to a fall of approximately 8 ft. to the lower level on an adjacent building and/or approximately 25 ft. to the ground floor.  Marfi Contracting Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1345099, citation number 2, item number 1 and was affirmed as a final order on 05-16-2019, with respect to a workplace located at W.T. Clarke High School, 740 Edgewood Dr., Westbury, NY 11590.
