OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    A.)   F & S Tool, Inc., 2300 Powell Avenue, Erie, PA  16506, on or about August 18, 2015 and times prior thereto:  Employees were potentially exposed to fire and explosion hazards from dust collectors located inside the building.  Dust collectors included, but not limited to, the Torit SDF 6 (Serial #: IG394319-001) were located inside the building, and were not equipped with devices to mitigate a potential dust explosion.  The dust collector collected dusts from materials including, but not limited to, aluminum oxide and stainless steel.      B.)   F & S Tool, Inc., 2300 Powell Avenue, Erie, PA  16506, on or about August 18, 2015 and times prior thereto:  Employees were potentially exposed to fire and explosion hazards from dust collectors that were not equipped with devices and systems to prevent the propagation of explosions and associated flame fronts from the dust collector through the associated dust collection system duct work.  Dust collection systems including, but not limited to, the Torit SDF 6 (Serial #: IG394319-001) was not equipped with approved devices to prevent the propagation of a dust explosion and associated flame fronts throughout the dust collection system and the associated duct work.  The dust collector collected dusts from materials including, but not limited to, aluminum oxide and stainless steel.     C.)   F & S Tool, Inc., 2300 Powell Avenue, Erie, PA  16506, on or about August 18, 2015 and times prior thereto:  Employees were potentially exposed to fire and explosion hazards from dust collectors located outside the building near an employee break area.  Dust collectors included, but not limited to, the Torit DFT2-8 (Serial #: IG363057-001) were located outside the building near the employee break area, and were not equipped with devices to mitigate a potential dust explosion.  The dust collector collected dusts from materials including, but not limited to, aluminum oxide and stainless steel.      D.)   F & S Tool, Inc., 2300 Powell Avenue, Erie, PA  16506, on or about August 18, 2015 and times prior thereto:  Employees were potentially exposed to fire and explosion hazards from dust collectors that were not equipped with devices and systems to prevent the propagation of explosions and associated flame fronts from the dust collector through the associated dust collection system duct work. F & S Tool, Inc., 2300 Powell Avenue, Erie, PA  16506, on or about November 9, 2015 and times prior thereto:  A set of wooden fixed industrial stairs was used to access a platform that contained equipment including, but not limited to, an air compressor unit.  The fixed industrial stair contained 15 risers, and was not equipped with a standard hand / guard rail on the left side of the stairs.
