29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a- Exterior Building: An employee was exposed to fall hazards of approximately 12ft as the scaffold work platform consisted of 2-3 planks as full planking was not utilized. Condition noted on or about 08/15/2015.        Darco Construction Corp was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.451(b)(1):  which was contained in OSHA Inspection Number 435196, Citation # 1, Item # 3, issued on 08/03/2012 with respect to a work place located at 58 Lafayette Road, Lot # 15, Livingston NJ 07039 which became a final order on 11/29/2012.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.    a- Exterior Building: An employee was exposed to fall hazards of approximately 12ft from the second scaffold section in which a means of scaffold access was not utilized. Condition noted on or about 08/15/2015. 29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a-Exterior Building: An employee was positioned on a bridged section of tubular welded frame scaffold in which a means of fall protection was not utilized exposing the worker to fall hazards of approximately 12ft to the concrete below. Condition noted on or about 08/15/2015.    b-Exterior Building: An employee was positioned at the unprotected end of a pump jack scaffold in which end rails were not utilized exposing the worker to fall hazards of approximately 12ft to the concrete below. Condition noted on or about 08/15/2015.
