29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material were not provided to permit easy handling of material without the operator placing a hand in the danger zone: On or about 08/12/15, at the location of 3801 S. Oliver in Wichita KS, in Hanger Post A9, an employee operating a drill press (Rockwell Sn#1445868) was exposed to amputation hazards in that a tool was not provided to remove material from the drill press table.

29 CFR 1910.178(a)(6): Nameplates or markings for powered industrial trucks were not in place or were not maintained in a legible condition: Employees engaged in material handling activities outside of the facility utilizing a Hyster-50 forklift (#31095524) were exposed to struck-by hazards in that the side shift control lever did not include markings which identified the direction of travel.
