29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): (a) Initiatives, LLC, DBA Squeegee Squad, Bromley Commons Condominiums, 350 Detroit Street, Denver, CO: On or about August 19th, 2015, the employer had not assessed the workplace prior to directing an employee to access the 3/12 pitch roof to wash windows. This condition exposed an employee to a fall hazard of approximately 14 feet. Abatement Note: Abatement certification is required for this item (see enclosed abatement certification letter).

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Initiatives, LLC, DBA Squeegee Squad, Bromley Commons Condominiums, 350 Detroit Street, Denver, CO: On or about August 19, 2015, the employer did not ensure employee(s) were provided with and using personal fall protection equipment while washing windows on a 3/12 pitch roof. This condition exposed employees to a fall hazard of approximately 14 feet. Abatement Note: Abatement certification is required for this item (See Enclosed Abatement-Certification Letter). Squeegee Squad was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.132(a), which was contained in OSHA inspection number 1072707, citation number 1, item number 2 and was affirmed as a final order on 8-6-2015, with respect to a workplace located at 251 East 12th Street, Denver, CO 80204.
