29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk shall be protected by a cover that leaves no openings more than 1 inch wide. a) On or about September 3, 2015 a floor hole created by spacing in unsecured wooden platforms was not covered and/or guarded to prevent employees from walking into the gap of 2 inches in the Saw Department 29.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about September 3, 2015 the bottom portion of the UMT panel saw blade was not guarded to prevent employees from contacting any portion of the rotating blade while it was operating while pushing door through it on the roller conveyor and as they walked on the wooden personnel platform.
