29 CFR 1910.212(b): Machines designed for fixed locations were not securely anchored to prevent walking or moving:  (a) Plant 2, Packing Area - The employer had a Hytrol conveyor, model 10,444, serial number 491011, used to convey plastic barrels of snacks.  This conveyor had a height from the floor to the underside of the conveyor of 27.75 inches, a width of 12 inches, and a length of 60 feet.  The conveyor also had legs with base plates with holes for securing the conveyor to the floor; however, the employer did not securely anchor the conveyor to prevent the conveyor from tipping over, on or about August 17, 2015.  On August 17, 2015, an employee attempted to swing underneath the conveyor feet first, which caused the conveyor to tip over and fall onto the employee, causing severe contusions to the head and face requiring hospitalization.  Corrected at the time of the inspection.
