29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: a) Bldg. 116 (3400 Lebanon Pike) - On or about 8/20/15, employees were exposed to tripping hazards when (2) 1-2 inch deep and approx. 1ft x 1ft width holes were not covered or filled in on a walking concrete/cement surface. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
