29 CFR 1926.502(d)(5): Snaphooks were not sized to be compatible with the member to which they were connected to prevent unintentional disengagement of the snaphook: Location: 186 Main Street, Weymouth, MA. Emplyees were exposed to fall hazards greater than 33 feet, while working on a flat roof, where the snaphook was connected to the HVAC perimeter base rail rigging holes, preventing the snaphook from latching properly.
