29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. On or about August 13, 2015, employees engaged in decking activities on new residential construction were using a board reinforced with plywood, and had propped against the roof's edge, to access the roof. This exposed the employees to struck-by hazards and/or fall hazards up to and greater than six (6) feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about August 13, 2015, employees engaged in decking activities greater than six (6) feet above ground level were not provided with any type of fall protection nor had the employer created and implemented a fall protection plan which demonstrated it was infeasible or created a greater hazard to use these systems. This exposed employees to fall hazards. Requena Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 943986, citation number 1, item number 1a, and was affirmed as a final order on December 3, 2013, with respect to a workplace located at 2817 Indian Grass Court, Edmond, Oklahoma 73013.

Requena Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 943986, citation number 1, item number 1b, and was affirmed as a final order on December 3, 2013, with respect to a workplace located at 2817 Indian Grass Court, Edmond, Oklahoma 73013.
