29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles: On or about June 12, 2020, the employer did not ensure that the employees were in use of eye protection while using a hammer and pneumatic nail guns to install the corner trims to the roof of a multi-family building. This exposed the employees to eye injuries from struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 12, 2020, the employer did not ensure that employees were in use of fall protection while working from the roof of a multi-family building installing trim. This exposed the employees to a fall hazard greater than six(6) feet to the ground below. Requena Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1086581, citation number 2, item number 1a, and was affirmed as a final order on June 24, 2016, with respect to a workplace located at 10605 Haskell Lane, Oklahoma City, Oklahoma 73162.
