OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from inadequate training on the use of below the hook lifting devices:        (a) Facility, 404 Sarah Furnace Road, Imler, PA:  On or about 08/05/15 an employee was using an overhead crane with a below the hook lifting device attached to move a steel I-beam.  The employee was attempting to set the beam on the floor and remove the device.  One side of the device arm was removed and the employee began to lift the crane hook up when the other arm caught the beam and caused it to fall over onto the employee, fracturing his right femur.  The employee was only employed at the facility for 2 days and had not received adequate training and instruction on the proper use and technique of working with below the hook lifting devices to move beams.
