29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) On or about at 8/21/15 located at 3715 Walton Way Extension, Augusta, GA: A employee was stocking the 2nd floor balcony roof with plywood on a apartment complex that was under construction while working at the edge, the employer did not ensure that fall protection was used by the employee while exposed to fall hazards of approximately 22 feet.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: On or about at 8/21/15 located at 3715 Walton Way Extension, Augusta, GA: Two employees were on the 3rd floor roof installing plywood on a apartment complex that was under construction while working and while having their harness D-ring secured to their waist instead of located in the center of the wearer's back near shoulder level. The employees were 35 feet from the lower ground level. The employer did not ensure that fall protection was used by the employees correctly while exposed to fall hazards.
