29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:    On or about August 24, 2015, employees were exposed to lead on surfaces including, but not limited to a bench in the clean room. Wipe samples taken from the clean room bench indicated the presence of lead at 752.54 ug/ft2.

29 CFR 1910.1025(i)(4)(iv): Employees entering lunchroom facilities with protective work clothing or equipment were not required to remove surface lead dust by vacuuming, downdraft booth, or other cleaning method(s):  On or about August 24, 2015, the employer did not provide vacuums or an alternate cleaning method for employees to remove lead dust from protective clothing prior to entering the lunchroom and locker room areas.

29 CFR 1910.1025(c)(1): Employee(s) were exposed to lead at concentrations greater than fifty micrograms per cubic meter of air averaged over an eight-hour period:    a)  On or about September 2, 2015, an employee working at Pot 18 was exposed to lead at an 8-hour Time-Weighted Average (TWA) of 69.525 µg/m3 during the 412 minute sampling period.  This is 1.39 times the Permissible Exposure Limit (PEL) of 50 µg/m3.  Exposure calculations include a zero exposure increment for the 68 minutes not sampled.       b)  On or about September 2, 2015, an employee working at Pot 17 was exposed to lead at an 8-hour Time-Weighted Average (TWA) of 71.01 µg/m3 during the 401 minute sampling period.  This is 1.42 times the Permissible Exposure Limit (PEL) of 50 µg/m3.  Exposure calculations include a zero exposure increment for the 79 minutes not sampled.     Victory White Metal Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1025(c)(1), which was contained in OSHA inspection number 922522, citation number 2, item number 4a and was affirmed as a final order on February 13, 2014, with respect to a workplace located at 6100 Roland Ave., Cleveland, OH, 44127.

29 CFR 1910.1025(e)(1)(i): For any employees exposed to lead above the permissible exposure limit for more than 30 days per year where engineering and work practice controls (including administrative controls) were feasible but did not reduce the employees' exposure to or below the permissible exposure limit, the employer did not use engineering and/or work practice controls to reduce exposures to the lowest feasible level:    a)  On or about September 2, 2015, an employee working at Pot 18 was exposed to lead at an 8-hour Time-Weighted Average (TWA) of 69.525 µg/m3 during the 412 minute sampling period.
