29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph. On or about August 21, 2015, at the job site located at 29651 Corporate Cir, Westlake, Ohio 44145; the employer did not ensure the live electrical conductors in a 240 volt control panel box, located on the Mast Climbing Work Platform KPM-8, were guarded against accidental contact.
