29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a). Masonry Grout Mixing Area - On or about and times prior to August 25, 2015, an employee was working from the top of a Quikrete Masonry Silo to release Portland Cement into the silo without a fall protection system in place, thus exposing employees to a fall hazard of approximately 14.3 feet onto concrete bricks and earthen ground.

Masonry Grout Mixing Area - On or about and times prior to August 25, 2015, an employee's arms and hands were not protected from Portland Cement when re-filling the Quikrete Masonry Silos and mixing cement, thus exposing employees skin injuries.
