29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): a. Hendrix Middle School, 4950 Hyde Road, Cumming, GA (Jobsite) - An employee was exposed to struck-by hazard. An employee was operating the Mitsubishi Sit-Down Forklift to haul the dismantle scaffold pieces. The employee was not provided with the proper forklift training on this particular equipment, on or about 06/26/20.
