29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): (a) Jobsite, on or about August 25, 2015 - employees that were performing work in an excavated trench that measured approximately 11 feet wide by 21 feet long were not protected from a cave-in hazard of this trench. The Northeast end of the trench ranged between 6 to 7 feet deep.
