29 CFR 1910.36(d)(1) as required by 1960.8 (b): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:   1. In building one west, on the second floor, the door leading out to the roof was locked with a dead bolt there by exposing employees to a fire hazard.  2. In the offsite warehouse, one fire exit leading out of the east side of the warehouse was locked with a deadbolt there by exposing employees to a fire hazard.  3. In the offsite warehouse, two fire exit leading out of the west side of the warehouse were locked with a deadbolt there by exposing employees to a fire hazard.

29 CFR 1910.219(e)(1)(i) as required by 1960.8 (b): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt:   In Building One West, in the sub-basement, the employer did not provide adequate guarding around horizontal belts on the Champion air compressor, Model 350.  Air Compressor is self activated and more than an eight inch section of the guarding is missing there by exposing employees to caught in hazards.

29 CFR 1910.305(a)(2)(ix) as required by 1960.8 (b): All lamps for general illumination shall be protected from accidental contact or breakage by a suitable fixture or lampholder with a guard.   1. Onsite warehouse, at the loading dock, two Phoenix docklite loading lights, Model # DL-42ARM, 150 VAC, used to illuminate trailers for loading and unloading were not properly guarded to prevent accidental contact or breakage. Employees were exposed to an electrical shock hazard.   2. Offsite warehouse, at the loading dock, one Wesco incandescent dock light, no model number, used to illuminate trailers for loading and unloading were not properly guarded to prevent accidental contact or breakage. Employees were exposed to an electrical shock hazard.

29 CFR 1910.305(g)(1)(iv)(A) as required by 1960.8 (b): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   At the offsite warehouse, at the forklift battery charging station, a Ferro 1500 series forklift battery charger was powered using flexible cable with plug direct from the electrical panel as a substitute for fixed wiring there by exposing employees to an electrical hazard.

29 CFR 1910.219(d)(1) as required by 1960.8 (b): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    In Building One West, in the sub-basement, the employer did not provide adequate guarding around on the Champion air compressor, Model 350.  Air Compressor is self activated and more than an eight inch section of the guarding is missing there by exposing employees to caught in hazards.    The Battle Creek VA Medical Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(d)(1), which was contained in OSHA inspection number 531579, citation number 2, item number 3 and was affirmed as a final order on February 11, 2013, with respect to a workplace located at 5500 Armstrong Road, Battle Creek, Michigan 49015.
