29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ingition may include open flames, lightning, smoking, hot sirfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: Near Building 32, the 250 gallon diesel and 250 gallon gasoline fuel tanks were not grounded to prevent static electric sparks which exposed employees to fire hazards.

29 CFR 1910.134(h)(3)(iii): Air and oxygen cylinders of self-contained breathing apparatus were not maintained in a fully charged state: In the Industrial Hygienist Lab, the air tank on a MSA Model 401 self-contained breathing apparatus maintained for emergency use was not fully charged. The 30 minute air tank contained approximately 12 minutes of air.
