29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  a) Intertek USA Inc., Arlington Heights, IL, did not ensure that employees required to wear North half-mask and full-face respirators, 3M half-mask respirators and 3M model(s) 6800 and FF 402 full-face respirators in operations including, but not limited to, chemical lab work, dry cleaning and the burn lab were provided with a medical evaluation to determine their ability to use a respirator.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.  a) Intertek USA Inc., Arlington Heights, IL, did not ensure that employees required to wear North half-mask and full-face respirators, and 3M half-mask and 3M model(s) 6800 and FF 402 full-face respirators, equipped with various chemical cartridges and/or particulate filters, when working in departments, such as the chemical labs, dry cleaning or LSS (fire room), were fit tested prior to initial use of the respirator(s), whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter.   Affected employees were exposed to tetrachloroethylene ("Perchlor"), lab chemicals, such as formaldehyde, tetrahydrofuran and acids, as well as other chemicals, such as acetone, alcohols, heptane and toluene used as ignition sources/accelerants, and smoke and particulates resulting from materials being burned, such as newspaper, wood and polyurethane.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) Intertek USA Inc., Arlington Heights, IL, did not ensure that employees who operate the Yale stand-up forklift in the warehouse were properly trained, evaluated and certified to perform tasks in a safe manner.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
