A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees:    a) On or about August 25, 2015 - at a jobsite located at 47th & St. Lawrence Street, Chicago, IL. -employees performing plumbing activities in a trench measuring four (4) feet, five (5 ) inches to six (6) feet, five (5 ) inches in depth were not provided with a safe means of egress, thereby exposing the employees to cave in hazards.    The Lakeshore Plumbing, Inc. was previously cited for a violation of this occupational safety and health standard or its  equivalent standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 871903, citation number 1, item number 2, and was affirmed as a final order on March 12, 2013, with respect to a workplace located at 3257 W. Pierce Street, Chicago, IL.    The Lakeshore Plumbing, Inc. was previously cited for a violation of this occupational safety and health standard or its  equivalent standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 984316, citation number 1, item number 2, and was affirmed as a final order on October 7, 2014 with respect to a workplace located at 2489 N.
