29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a. On or about December 23, 2017, at the above addressed jobsite, employees were installing new water main in a trench measured at five (5) feet four (4) inches in depth. The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards. Lakeshore Plumbing Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1087834, citation number 1, item number 2 and was affirmed as a final order on December 29, 2015, with respect to a workplace located at 47th & Saint Lawrence St., Chicago, IL. Lakeshore Plumbing Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 984316, citation number 1, item number 3 and was affirmed as a final order on October 7, 2014, with respect to a workplace located at 2489 N.
