29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a. In the production area where mud flaps were made, the shear on the plastic extrusion line 1 did not have guards in place to prevent employees from placing any part of the body in the point of operation.   b. In the production area where mud flaps were made, the shear on the plastic extrusion line 2 was not adequately guarded in that employees were able to reach around and under the guard in place.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Located at Deflecto in Dover, OH:  a. On the 2015 OSHA 300 log the employer did not include all the details required in column F for each entry.      1. Case number 98, CM on 5/6/15, did not include what the injury was and what part of the body was affected.     2. Case number 99, DD on 5/27/15, did not include what the injury was and what part of the body was affected.      3. Case number 105, DH on 6/27/15, did not include what caused the shoulder strain.      4. Case number 103, RB on 6/28/15, did not include what the injury was and what part of the body was affected.      5. Case number 102, CC on 7/1/15, did not include what the injury was and what part of the body was affected.     6. 29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  Located at the Deflecto in Dover, OH:  a. The employer did not report a finger tip amputation of a temporary employee that occurred on or about August 14, 2015 within twenty-four hours of the incident.
