On or about August 27, 2015, the employer did not perform an initial assessment of the Lead exposure to employees removing paint using an abrasive blasting method from the water tower located at 1149 East Chestnut Street Chilton, Wisconsin.

29 CFR 1926.62(h)(1): "All surfaces shall be maintained as free as practicable of accumulations of lead". Employees removing Lead-based paint from a water tower using an abrasive blasting method were exposed to Lead from contaminated surfaces in the decontamination trailer. a. Wipe samples of the inside door handle of the exit door of the "clean" side of the decontamination trailer had a Lead level of 31.785 ug/16 in2 which equates to 286 ug/ft2. b. Wipe samples of shower side door handle of the door between the showers and the "clean" side of the decontamination trailer had a Lead level of 77.255 ug/16 in2 which equates to 695 ug/ft2. c. Wipe samples of the top surface of a bench in the "clean" side of the decontamination trailer had a Lead level of 138.140 ug/16 in2 which equates to 1243 ug/ft2.

29 CFR 1926.62(i)(1): The employer shall assure that in areas where employees are exposed to lead above the PEL without regard to the use of respirators, food or beverage is not present or consumed, tobacco products are not present or used, and cosmetics are not applied. a. In the ARS Decontamination Trailer by the water tower located at 1149 East Chestnut Street Chilton, Wisconsin, two water bottles were observed in a contaminated changing trailer. Cigarette butts were observed on the ground outside of the trailer. "Ghost Wipe" samples taken from inside and outside of the trailer indicated elevated Lead levels.

At the jobsite located at 1149 East Chestnut Street Chilton, Wisconsin, the employer did not provide adequate shower facilities to employees exposed to Lead. Decontamination trailer observed on September 9, 2015 did not have the supplied Aluminum folding stairs in place by the "dirty" side door requiring the employees to enter and exit through the "clean" side to access the showers.

Employees were exposed to Lead while drinking water in the decontamination trailer and smoking cigarettes on the job site. Water bottles were inside the decontamination trailer and cigarette butts were on the ground near the decontamination trailer.
