29 CFR 1926.405(b)(1): Openings in boxes, cabinets, and fittings through which conductors entered were not effectively closed. On August 27, 2015, and times prior to, employees were working near an electrical box that was not effectively closed. This condition exposed employees to electrical shock hazards.

29 CFR 1926.1101(h)(2)(i): The employer failed to implement a respiratory protection program in accordance with 29 CFR 1910.134(b) through (d) [except (d)(1)(iii)], and (f) through (m), which covers each employee required by 29 CFR 1926.1101 to use a respirator: 29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve. On August 27, 2015, during their break times employees were not storing their respirators to protect them from damage. This condition exposed employee to health hazards. The MFH Environmental Corp. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.134(h)(2)(i), which was contained in OSHA inspection number 108104, citation number 2, item number 1/d and was affirmed as a final order on 02/29/2012, with respect to a workplace located at 2150 E. Evans Ave. Denver CO, 80208.
