On or about January 26. 2021, the Force Air 2000 EC air exhaust units were connected using more than one power extension cords connected together (Daisy chain connection) that were not installed in accordance to the manufacture's listed label use. This condition exposed employees to electrical shock and /or burns.

29 CFR 1926.404(b)(1)(iii): Assured equipment grounding conductor program. The employer shall establish and implement an assured equipment grounding conductor program on construction sites covering all cord sets, receptacles which are not a part of the building or structure, and equipment connected by cord and plug which are available for use or used by employees. On or about January 26. 2021, an equipment grounding conductor program was not developed and implemented at the construction jobsite that covers all flexible cord sets that connect the Force Air 2000 EC air exhaust units being used. This condition exposed employees to electrical shock and /or burns.

On or about January 26. 2021, the Force Air 2000 EC air exhaust units were connected using power extension cords without strain relief or strain relief that was damaged.
