29 CFR 1910.38(e): Training. An employer must designate and train employees to assist in a safe and orderly evacuation of other employees.    a)  The employer had not effectively trained employees working in plant regarding evacuation from the facility in a safe and orderly fashion.  On or about August 27, 2015 during the combustible metal dust fire that activated the evacuation from the plant employees did not completely evacuate to a safe location and exited the door next to the fire.

29 CFR 1910.156(c)(1): The employer did not provide training and education commensurate with those duties and functions that fire brigade members were expected to perform:    a)  The employer did not perform any training or education to the maintenance employees responding as a fire brigade to attempt to extinguish the fire with the dust collector involving aluminum metal dust on August 27, 2015 commensurate with the duties they were expected to perform.

29 CFR 1910.157(g)(4): Employees who have been designated to use fire fighting equipment as a part of the emergency action plan were not provided training in the use of appropriate equipment upon initial assignment and at least annually thereafter:    a)  The employer had not provided documented training to the employees designated to respond to fires including proper use of extinguishers since 2013.  Some of these employees responded to a metal dust fire on August 27, 2015 exhausting several extinguishers attempting to extinguish the fire before the local fire department was contacted to respond.

29 CFR 1910.165(b)(2): The employee alarm could not be perceived above ambient noise or light levels by all employees in the affected portions of the workplace:    a)  The fire alarm sounded to evacuate the facility on August 27, 2015 during a combustible aluminum dust fire was not able to be heard by all employees in the facility to effectively evacuate the facility to a safe location.
