An employer must designate and train employees to assist in a safe and orderly evacuation of other employees. a) The employer had not effectively trained employees working in plant regarding evacuation from the facility in a safe and orderly fashion. On or about August 27, 2015 during the combustible metal dust fire that activated the evacuation from the plant employees did not completely evacuate to a safe location and exited the door next to the fire.

29 CFR 1910.157(g)(4): Employees who have been designated to use fire fighting equipment as a part of the emergency action plan were not provided training in the use of appropriate equipment upon initial assignment and at least annually thereafter: a) The employer had not provided documented training to the employees designated to respond to fires including proper use of extinguishers since 2013. Some of these employees responded to a metal dust fire on August 27, 2015 exhausting several extinguishers attempting to extinguish the fire before the local fire department was contacted to respond.
