29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition:  (a)    On or about October 12, 2017, on the east side of Furnace One and adjacent to the Tapwell, there was a floor hole in the concrete walking-working surface which measured approximately 2 ft. X 1 ft. X 3.5 in. (depth).  (b)    On  or about October 12, 2017, on the north side of the  Die Cast #24 machine, there was a floor hole in the asphalt walking-working surface which measured approximately 21 in. X 7 in. X 2 in. (depth).  (c)    On or about October 12, 2017, on the north side of the holding tank transformer for Die Cast #14 machine , there was a floor hole in the asphalt walking-working surface which measured approximately  1 ft. X 1 ft. X 3 in. (depth).  (d)    On or about October 12, 2017, on the south side of the holding tank transformer for Die Cast #14 machine , there was a floor hole  in the asphalt walking-working surface which measured approximately  17 in. X 12 in. X 2.25 in. (depth).

29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident:   During 2017, multiple incidents occurred in which operator(s) of forklift trucks were involved in accidents or near-miss incidents. No refresher training was provided to the operators of the forklifts after the incidents.  (a) On or about June 20, 2017 (Incident #2160), an employee in the Melt Department was involved in an accident while operating a forklift truck to pick up scrap carts from behind Die Cast Machine #30. The operator backed up the forklift into a parts basket and pushed it into a guard rail surrounding the lube system, causing leakage of that system.  No refresher training on relevant topics was provided to the operator involved in the accident.  (b) On or about March 15, 2017 (Incident #2406), an employee was using a forklift to pull scrap tray from under the trim press on Die Cast Machine #16 and bumped into the work table. No refresher training on relevant topics was provided to the operator involved in the accident.  (c) On or  about March 11, 2017 (Incident #1919), an employee used a forklift to swap out scrap cart at Die Cast Machine #35. When placing the new hopper, the forklift operator struck guarding (cage) around robot, causing damage to the cage. No refresher training on relevant topics was provided to the operator involved in the accident.  (d) On or about February 2, 2017 (Incident #1846), an employee used a forklift to switch out a scrap cart on Die Cast Machine #33 trim press.  The employee moved an empty basket to access the area and the basket being moved contacted an adjacent row of baskets which were stacked three (3) high and caused the top basket to tip over. No refresher training on relevant topics was provided to the operator involved in the accident.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  (a)    On or about October 6, 2017, in the Melt Department, powered industrial truck # B4134 had multiple cracks in its windshield. This truck was not been taken out of service, and/or repairs were not made by authorized personnel.  (b)     On or about October 6, 2017, in the Melt Department, powered industrial truck # T193746 had a pouring alarm and a front blue safety light which were not working; the tires on this truck had excessive wear. This truck was not taken out of service and/or repairs were not made by authorized personnel.   (c)     On or about October 6, 2017, in the Melt Department, powered industrial truck # T193744 had a pouring alarm which was not working. This truck was not taken out of service and/or repairs were not made by authorized personnel.
