29 CFR 1910.303(e)(1)(ii): Electrical equipment did not have markings giving voltage, current, wattage, or other ratings as necessary:  a)  On or about September 15, 2015, the Hoffman electrical enclosure located next to the vehicle exit door in the Drop-Pit Drive-Through building was not labeled with voltage, current, and wattage.  b)  On or about September 15, 2015, the Furnas electrical enclosure located in the vehicle exit area in the Drop-Pit Drive-Through building was not labeled with voltage, current, and wattage.  DOOR COUNTY COOPERATIVE DBA SHIRLEY FEED MILL WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1910.303(e)(1)(ii), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 109849, CITATION NUMBER 2, ITEM NUMBER 2A AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 7, 2013 WITH RESPECT TO A WORKPLACE LOCATED AT 3809 SHIRLEY ROAD, DE PERE, WISCONSIN 54115.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  a) On or about August 27, 2015, the employer did not have markings to identify the purpose of switches installed in the Hoffman electrical enclosure located next to the vehicle exit door in the Drop-Pit Drive-Through building at the main site.  b) On or about August 27, 2015, the employer did not have markings to identify the purpose of switches installed in the Furnas electrical enclosure located in the vehicle exit area in the Drop-Pit Drive-Through building at the main site. 29 CFR 1910.1200(h)(3)(iv): Employees had not been provided with GHS 2012 training including an explanation of the labels received on shipped containers and the workplace labeling system to be used by their employer; the safety data sheet, including the order of information, and how employees can obtain and use the appropriate hazard information:  a) On or about September 15, 2015, the company had not provided training to all affected employees regarding the location and means to obtain information on the Hazard Communication Program and Safety Data Sheets (SDS's).  b) On or about September 15, 2015, the company had not provided training to all affected employees regarding an explanation of the current labeling system and details of the Global Harmonization labeling requirements.
