29 CFR 1910.119(d)(2)(i)(D): Information concerning the technology of the process did not include safe upper and lower limits for such items as temperatures, pressures, flows or compositions: 1) On August 24, 2015, and at prior times, the employers process safety information pertaining to the ethane system did not include safe upper pressure limits for the vaporizers. Calculations for the safe upper pressure limit for the vaporizers were based on the pressure rating of a flange which was not an integral part of the vaporizer.

29 CFR 1910.119(d)(3)(i)(B): Process safety information pertaining to the equipment in the process did not include the piping and instrument diagrams (P&ID's): 1) On August 24, 2015, and at prior times, the P&ID was inaccurate or did not represent existing equipment that was a part of the process in the following instances: a) where the nitrogen pressure regulators for the ethane flow control valves located on the stanchions were not listed on the P&ID. b) where the 250 psi pressure safety valves (PSV 648A and PSV 648B), located at the second state regulator panel, were labeled set at 150 psi . c) where the nitrogen line pressure control valve (PCV 6420) was labeled set at 65 psi , which did not reflect current field settings.

29 CFR 1910.119(l)(5): Where a change covered by this paragraph resulted in a change in the operating procedures or practices required by paragraph (f) of this section, the employer did not ensure that such procedures or practices were updated accordingly: 1) On August 24, 2015, and at prior times, the employer did not ensure that standard operating procedures were updated due to a required management of change in the following instance: a) On February 24, 2014, the employer implemented a management of change related to the installation of a temperature monitor, alarm, and transmitter for the ethane line located after the second stage regulator. This device was installed to provide low temperature protection on the ethane system to protect the piping from any liquid ethane that might pass the vaporizers. The employer did not ensure that standard operating procedures were updated with changes due to this new device.
