29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): (a) On or about August 25, 2015 at worksite 1375 W Snyder Blvd, Nixa, Missouri.-employees in the parts cleaning area were exposed to prolonged chemical contact while cleaning parts due to the lack of protective equipment when handling Safet-Kleen Premium Solvent. Employees were not given personal protection as per MSDS, they were witnessed handling solvent without using of required skin, face or splash protection. Abatement Note: Abatement certification is required for this item use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) On or about August 25, 2015, at worksite 1375 W Snyder Blvd, Nixa, Missouri.- employees were exposed to hazard of electrical shock from exposed conductors in a flexible cord which had broken free of its connection to a grinder on the in fabrication area. Abatement certification is required for this item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. .
