29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite-specific procedures was not established and implemented for required respirator use: At the industrial worksite located at 1375 West Snyder Boulevard in Nixa MO 65714: Employees were exposed to respiratory illnesses caused by respirable dust from handling expandable flake graphite 3538, as well as dermatitis, and other undetected medical conditions. This was most recently documented on or about March 6, 2023, and at times prior when employees were required to wear 3M N95 Particulate Respirators when working in the lube stick department. The employer failed to implement a written program to cover such operations prior to employees' use of a respirator.

29 CFR 1910.303(g)(1): Sufficient access and working space were not provided and maintained for all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: At the industrial worksite located at 1375 West Snyder Boulevard in Nixa MO 65714: Employees were exposed to potentially hazardous electrical conditions while working in the lube stick department due to an electrical box equipment that was blocked with a five-gallon drum, placed in front of the equipment making it not readily accessible.
