29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   The employer does not ensure that eye protective equipment is used when operations present eye or face injury from physical agents.  This violation was observed on or about August 28, 2015 on the south side of the jobsite where an employee was exposed to struck-by hazards when using a nail gun for framing without eye protection.    Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that eye protective equipment is used when operations present eye or face injury from physical agents.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:   The employer does not ensure that supported scaffold poles bear on base plates and mud sills or other adequate firm foundation.  This violation was observed on or about August 28, 2015, on the south side of the jobsite where employees were exposed to a fall hazard when performing framing activities without base plates and mud sills on the scaffold poles.      Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that supported scaffold poles bear on base plates and mud sills or other adequate firm foundation.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   The employer does not ensure that each employee on a scaffold more than 10 feet above a lower level is protected from falling to the lower level.  This violation was observed on or about August 28, 2015, on the south side of the jobsite where employees were exposed to a fall hazard greater than 10 feet when performing framing activities without a fall protection system.     Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee on a scaffold more than 10 feet above a lower level is protected from falling to the lower level.

29 CFR 1926.452(j)(1):  Pump jack brackets, braces, and accessories were not fabricated from metal plates and angles. Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage.   The employer does not ensure that pump jack braces are fabricated from metal plates and angles.  This violation was observed on or about August 28, 2015, on the south side of the jobsite where the top of the pump jack was secured with wood exposing employees to a fall hazard when performing framing activities.       Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that pump jack braces are fabricated from metal plates and angles.

This violation was observed on or about August 28, 2015, on the south side of the jobsite where employees were exposed to a fall hazard when performing framing activities without the poles secured to the structure at the bottom.       Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that poles are secured to the structure by rigid triangular bracing or equivalent at the bottom.
