29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  On 8/31/15, at the Norcross facility, the employer did not create and implement specific written procedures for equipment being serviced and maintained at the facility.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(5)(ii): Lockout devices and tagout devices were not singularly identified.  On 8/31/155, in the Norcross facility, the employer did not ensure employees applying lockout locks, for service and maintenance of equipment, were identified properly.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts, flying chips and sparks.  On 8/31/15, in the production area, employees are exposed to ingoing nip points and rotating parts as the following areas were without guarding:  a - Outfeed side of Divider conveyor b - Rounder divider (flipping part) c - Intermediate Proofer    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
