29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  Masada Bakery LLC, Norcross, GA - In the wash room, employees were spraying cleaning solutions such as FS Foaming Smokehouse and Apex Process Cleaner, classified as corrosive. Employees were provided with regular impact resistance safety glasses. Chemicals resistant goggles were not provided and required, on or about 8/31/15.

The personal protective equipment hazard assessment was not certified in writing, on or about 8/31/15.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a)  Masada Bakery LLC, Norcross, GA - In the wash room, employees were spraying cleaning solutions such as ZEP-A-One, FS Foaming Smokehouse and Apex Process Cleaner, classified as corrosive. The eye wash in the room was not connected to a working water line, on or about 8/31/15.  b)  Masada Bakery LLC, Norcross, GA - In the warehouse storage area, a 300 gallons sodium hydroxide tote was in place. An emergency response team was established to response in the event of a spill. A suitable eye wash station was not connected to a working water line provided, on or about 8/31/15.

The information in Appendix D was not provided, on or about 8/31/15.
