On or about August 27, 2015, at 5020 Cromwell Dr., Kyle TX, the employer failed to ensure that the guardrail system installed was erected along the open sided platform. The guardrails were erected with a sizeable gap between the end of the work platform and the railing. Employees were exposed to a fall hazard of approximately 24 feet.

For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall: On or about August 27, 2015, at 5020 Cromwell Dr., Kyle TX, the employer failed to ensure the boards were erected to prevent tools and materials from falling on workers below. Employees on the pump jack scaffold were using pneumatic nail guns and hardy plank siding. Employees exposed to the hazard of being struck by material and/or the air guns falling approximately 24 feet.
