29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through. On or about May 19, 2015, in the Old Dip Room, an employee was exposed to a fall hazard from a floor hole, 7 1/2 inch by 5 1/2 inch square, on a metal plate flooring surrounding a resin tank and dip tank, while walking on the metal plate flooring to check on a circulation pump to the chiller a part of his left steel-toed shoe went into the hole and caused him to fall to the ground.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. The employer did not report within 24 hours an in-patient hospitalization involving employee who was injured at Siemens on May 19, 2015, and admitted to the hospital on May 19, 2015 and discharged on May 20, 2015.
