29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. At this demolition site, the competent person designated by the employer did not conduct regular and frequent inspections of heavy equipment used at this site. A piece of rebar was used instead of a suitable bolt or pin to secure a hinge connecting the excavator's bucket to the stick and boom of the excavator.
