Plugs were missing on at the following locations:      1) Inlet to Vessel (V-1) below Pressure Safety Valve (PSV-12).      2) Level Control Valve (LCV-11) to Vessel (V-11) and outlet from Vessel (V-11).

29 CFR 1910.119(d)(3)(i)(B):  Information pertaining to the equipment in the process did not include accurate piping and instrument diagrams (P&ID's):  a) On or about October 1, 2105, and times prior the employer's piping and instrumentation diagrams (P&ID) were not accurate in that equipment information was found during the walkthrough that was not accurate on the P&ID. Process Safety Valve (PSV-14) was set to release at 960 psig but the P&ID identified the release setting at 1000 psig.
