29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) In Die Cast, Furnace #19 had a damaged electrical disconnect which required employees to de-energize in a different location. The energy control procedures were not up-dated to outline the new energy isolating device exposing employees to inadvertent/accidental restart of equipment. (b) In Die Cast, Furnace #24 had a damaged electrical disconnect which required employees to de-energize in a different location. The energy control procedures were not up-dated to outline the new energy isolating device exposing employees to inadvertent/accidental restart of equipment.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: (a) In the Truck Maintenance area, the air gun used to clean off the powered industrial vehicles had an air pressure of approximately 90 psi exposing employees to air embolism. (b) In the Second Operations, West Room, the air gun used by the Okuma #5 used to blow off parts had an air pressure of approximately 60 psi exposing employees to air embolism. (c) In the Second Operations, West Room, the air gun used by the Brother #4, Brother #2, and Okuma #3 used to blow off parts had an air pressure of approximately 50 psi exposing employees to air embolism.
