29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provided for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):    (a) The Ramos Roofing Company, at 1232 S Hover Street, Longmont, CO: On and preceding 8/31/15 the employer did not initiate and maintain a safety program which provided for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them).  Employees performed roofing activities on the Shops E building at the Village at Twin Peaks shopping center.  On 8/31/15 an employee prepping the steel decking for installation of insulation and membrane fell through an approximate 3 foot by 5.5 foot hole in the decking.  The employer did not determine the presence of the floor hole through an inspection of the jobsite and correct the hazard.  This condition exposed the employee to an approximate fifteen foot fall hazard. 29 CFR 1926.56(a): While work was in progress, construction areas, ramps, runways, corridors, offices, shops and storage areas were not lighted to the minimum illumination intensities listed in table D-3 of this subpart:     (a) The Ramos Roofing Company, at 1232 S Hover Street, Longmont, CO: On 8/31/15 while work was in progress, construction areas, ramps, runways, corridors, offices, shops and storage areas were not lighted to the minimum illumination intensities listed in table D-3 of this subpart.  Employees performed roofing activities on the Shops E building at the Village at Twin Peaks shopping center.  On 8/31/15 at approximately 6:05am an employee prepping the steel decking for installation of insulation and membrane fell through an approximate 3 foot by 5.5 foot hole in the decking.  The hole was located on the east side of the flat roof adjacent to an approximate eight foot tall parapet.  Sunrise was at 6:26am.  The work area was not provided with at least five foot candles of illumination.  This condition exposed the employee to an approximate fifteen foot fall hazard.

29 CFR 1926.501(b)(4)(ii): Employer did not ensure that each employee on a walking/working surface was protected from tripping in or stepping into or through holes (including skylights) by covers:    (a) The Ramos Roofing Company, at 1232 S Hover Street, Longmont, CO: On and preceding 8/31/15 the employer did not ensure that each employee on a walking/working surface was protected from tripping in or stepping into or through holes (including skylights) by covers.  Employees performed roofing activities on the Shops E building at the Village at Twin Peaks shopping center.  Employees prepped and installed insulation and membrane around ducting for rooftop air handling units (RTU's).  Dimensions of paired holes for each RTU were fourteen inches by twenty inches and eighteen inches by nineteen inches.  Two of the paired holes did not have covers over the holes.  This condition exposed employees to a fall hazard.

The decking hole had been covered by a four foot by eight foot sheet of OSB.  The employer did not ensure that the cover that had covered the hole was capable of supporting twice the weight of employees, equipment, and materials.  This condition exposed the employee to an approximate fifteen foot fall hazard.

On 8/31/15 an employee prepping the steel decking for installation of insulation and membrane fell through an approximate 3 foot by 5.5 foot hole in the decking.  The decking hole had been covered by a four foot by eight foot sheet of OSB.  The sheet of OSB that had covered the hole was not secured to the decking.  This condition exposed the employee to an approximate fifteen foot fall hazard.
