29 CFR 1910.178(m)(1): Trucks shall not be driven up to anyone standing in front of a bench or other fixed object. a) Worksite, Wet End Coating Area: Employee operating Toyota forklift (Model #8FGU20, Serial #64484) drove up to and struck employee doing paper work in front of a fixed station control panel; on or about 9/2/15. NOTE: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement of this violation as normally required by 29 CFR 1903.19.

29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing. a) Worksite, Wet End Area: Employee operating Toyota forklift (Model #8FGU20, Serial #64484) to transport a pallet of adhesive roll (approximately 68 inches in height, 40 inches in diameter) did not travel with the load backward causing him to strike another employee doing paper work at a station control. The operator's view was obstructed; on or about 9/2/15 NOTE: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement of this violation as normally required by 29 CFR 1903.19.
